Dr. Lane is an out of network provider. The FOCUS Center for Vision and Learning, LLC does not accept any insurance at this time. All appointments are paid for at the time of the visit. We will happily provide you with a receipt stating procedure codes and diagnostic codes needed to submit to your insurance provider. All claim submission is down by the patient. Depending on your specific policy, reimbursement will vary and will be sent directly to you by the insurance company. Dr. Lane cannot accept payment from the insurance company. If your insurance provider requires prior authorization, we would be happy to assist by filling out any necessary paperwork.
